Reliance Medical Associates provides Family Practice and Internal Medicine primary care. We are located at 107 Glen Oaks Blvd., Suite 200, in Hendersonville, Tennessee.
Dr. Margaret Burnett Ikard specializes in Internal Medicine and is certified by The American Board of Internal Medicine.
Dr. Michael S. Kellogg's specialty is Family Medicine and is certified by the American Board of Family Practice.
Dr. Jennifer L. Montague specializes in Family Medicine and Women's Health. She is also certified by the American Board of Family Practice.
Together with the entire Reliance Medical Associates team, Drs. Ikard, Kellogg, and Montague provide quality healthcare with a personal touch in a friendly, caring environment.

For other urgent issues after hours, please call our office at 615-822-1993 and then dial 123 when prompted. Your call will be forwarded to the Doctor on call.

Questions & General Info
We always welcome calls from our patients with questions regarding any aspect of their healthcare. Please understand that we may be with another patient and be unable to take your call at that moment. Leave us a message and we will return your call as soon as possible.

If your issue is urgent, please explain the circumstances in your message so that we may respond appropriately.

If your situation is a MEDICAL EMERGENCY, please advise us in your message and CALL 911 or go immediately to the nearest HOSPITAL EMERGENCY ROOM.

Patient appointments are scheduled from 8:00am to 12:00pm and 1:00pm to 5:00pm Monday through Friday. We are closed for lunch each day from 12:00pm to 1:00pm.

If you are unable to keep your appointment and must cancel or reschedule, please contact us so that we may schedule that time for another patient. We kindly ask for 24 hours notice of any cancellation whenever possible.

Please bring any current prescription and/or over-the-counter medications, vitamins, or supplements with you to your appointment.

If you believe your situation is a MEDICAL EMERGENCY, CALL 911 or go immediately to the nearest HOSPITAL EMERGENCY ROOM for treatment. We ask that you contact us as soon as possible thereafter so that we may follow-up and assist in your care.
Insurance and Payment Policy
Most major health insurance plans are accepted including Blue Cross, Medicare, Medicaid, Tennessee Healthcare, and others. Health insurance plans change frequently. Please call our office to confirm if your plan is among those we accept.

You should contact your plan provider with questions regarding specific coverage provided by your plan.

Co-pays, deductibles, and any expenses not provided by your medical insurance plan are expected at the time of your appointment.

Charges for any laboratory tests, imaging and radiology, specialist expenses, or any other costs not specifically provided by your insurance plan are your responsibility to coordinate payment with the provider.

Co-Pays and deductibles are due at the time of your appointment.
We ask you to arrive 10 to 15 minutes prior to your appointment so that your records may be updated.
Lab results are returned directly to our office. Once we have analyzed the test results, we will contact you.
Prescription/Refill requests for pain medications, antibiotics, and any controlled substance REQUIRE an office visit.
Any work-related visit requires a Workmans Compensation Authorization be provided at the time of the appointment.
