Thank you for visiting our website. Cookeville Primary Care Associates is open and seeing patients from age 5 and up. Office phone number: (931) 528-5787. We are located at 455 N Cedar Ave, Cookeville. Entrance and parking are at the rear of the building.

We will be trying a new concept in patient appointments, and make same day or next day appointments if requested. Hopefully, this will allow us to serve your medical needs better as we will be able to see you when you need to be seen, and not at some point in the distant future.
